TWO ORDER & BILLING OPTIONS
PROVIDER
A medical provider (or staff member) can place an order on behalf of the patient. In these cases, the office will be billed upfront for the order. The office can then charge the patient for the service. We recommend a maximum charge of $289. Please contact us for pricing details. This model ensures offices are compensated for the time involved with the evaluation and placement of the order.
All tests will include a professional interpretation provided by a board-certified pediatric sleep specialist

PATIENT ORDER
Patients can be directed to order the test directly from the website. In these cases, the patient will be paying for the service up front at a cost of $289. The order form will request their provider’s fax number. Results will be communicated directly to the patient and faxed to the provider.
